Medical Insurance Info

All of Avanti’s clinicians are in-network providers for Premera Blue Cross.

Dr. Bob Smith is an in-network provider for Cigna.

Dr. Diane Shin, Dr. Cheryl D’Ettorre, Dr. Moriah Boggs & Dr. Bob Smith are in-network providers for Aetna.

If Premera, Cigna or Aetna is your secondary insurance, Avanti cannot accept it at this time(client will have to pay Avanti in full and request a superbill in order to get reimbursed by insurance).

Avanti is an out-of-network provider with other insurance companies. This means that full payment is made to Avanti at the time of service and then the client submits a superbill provided by Avanti to their insurance company for reimbursement.  Please check with your Benefits Representative for your specific coverage.

“In-network” does not guarantee payment from your insurance. A deductible is the amount that you are liable for before your insurance plan begins paying. Insurance plans can vary greatly, so it is your responsibility to know your deductible and coverage. Please ask the following questions of your insurance company before scheduling so that you are properly informed:

Does my insurance plan cover therapy service codes 90791 & 90837?
What is my deductible and has it been met?
What is my co-pay or coinsurance per visit?
Do I have out-of-network benefits?

Payment Policy

Payment is required at time of service by credit card (Visa, MasterCard or American Express) or check.

Cancellation Policy

A minimum of 24 hours notice is required to cancel or postpone an appointment. Full payment is required for late cancellations or no-shows.

Due to the time commitment made to you, we ask that you provide 24 hours notice if you need to cancel your appointment or reschedule to avoid charges for missed sessions.  Voicemail, email and text can be utilized 24 hours a day in the event that an appointment needs to be rescheduled.

Fee Schedule

We offer a free initial 30-minute consultation(typically over the phone).

Individual or family (couples) for a 50-minute session is $180.

Individual or family (couples) for a 90-minute session is $220.

Initial intake session is $220. Our first meeting is an extended session so that a comprehensive evaluation can include your history, significant life experiences, and a clear picture of your current issues. This information will help us devise a plan to address your therapeutic needs and determine what schedule will best support your goals.

Group Therapy is $60 per session.

Support groups and classes are $40 per session.

Prices for all services are subject to change without notice, and current clients will be given a 30 day notification of any pending fee increases prior to them taking effect. 

Fee Schedule for seeing an Intern

This information will help us devise a plan to address your therapeutic needs and determine what schedule will best support your goals.

Assessments

For our comprehensive evaluations, we bill $325 for the initial intake session and then $225 per hour for each additional hour. Our evaluations can take a total of 15 to 20 hours, although there are situations in which it may take less or more time. 

Insurance companies typically do not cover educational or academic testing as they are not considered to be a “medically necessary” service. We charge a flat rate of $550 when families wish to include academic or giftedness testing as part of the evaluation. This includes the administration and scoring of the tests and report writing. Families who are interested in exploring possible learning disorders (e.g., dyslexia, math disorder, writing challenges, etc.) or want to learn about their child’s academic strengths and challenges will need to include academic testing in their evaluation. Our psychologists can also provide additional guidance and recommendations about academic testing during the initial parent appointment for families who are unsure about whether their child may need academic testing included.

Your insurance may or may not cover some or all of your assessment. Please reach out to your insurance to check your in-network benefits, out-of-network benefits, coverage, and limitations for psychological and neuropsychological testing. For clients paying out-of-network, we also provide superbills to clients who wish to submit it to their insurance companies for possible reimbursement.

We commonly bill the following codes for evaluations:

  • Initial Intake Appointment (CPT code 90791)

  • Testing Appointment (CPT code 96136 for the first 30 minutes and 96137 for each additional 30 minutes)

  • Neuropsychological Testing Services (CPT code 96132 for the first hour and 96133 for each additional hour)

  • Psychological Testing Services (CPT code 96130 for the first hour and 96131 for each additional hour)

Special Notes:

  • We don’t do custody assessments or assessments for children whose parents are currently undergoing divorce proceedings.

  • We don’t do assessments for children younger than 6 and adults over 40 years of age.
